for Retreats, Families & Volunteers
A Waiver of Liability Form MUST be completed by all Deer Run attendees or volunteers.
GROUP LIABILITY WAIVER
This waiver is REQUIRED for each individual attending a group retreat.
Download and complete the form — either print or electronic. Then give or email the completed form to your group leader before your retreat date(s).
Group Leaders: Put completed forms in alphabetical order and give to your greeter upon arrival at Deer Run.
FAMILY LIABILITY WAIVER
This waiver is REQUIRED when multiple members of the same family (or friends) are attending a family event.
One adult person is signing responsibility for each adult and minor listed on the waiver.
VOLUNTEER LIABILITY WAIVER
This waiver is REQUIRED by anyone serving as a volunteer at Deer Run.
An adult must sign the waiver if the volunteer is a minor.
